The time and place of this cover photo is said to have been in Chicago on January 24th, 1982. Here is one reference:

I just now saw that this info was already stated at the main article for the album. The exact quote from the article is:
- "The album's cover photo was taken at a performance in Rosemont, Illinois on 24 January 1982, by photographer Paul Natkin."
