The National Portrait Gallery images give his rank as Lieutenant Colonel, as does this entry at bosleys.net: [1]. The Later also provides the citation for his DSO:

"The citation for the DSO appeared in the London Gazette on the 7th March 1918. “For conspicuous gallantry and devotion to duty when commanding his Battalion. At a most critical period of an attack he rallied his men and led an advance on an enemy strong point which was taken and held. He organised the defence of the ground gained exposing himself under heavy fire for some hours, with an utter disregard of personal safety. Had it not been for his initiative at a difficult time no ground would have been gained at this point.” 3rd Baron Swansea, Lieutenant Colonel Odo Richard Vivian was born in 1875, He was first commissioned into the Glamorgan Yeomanry and at the time of the award of the DSO was Commanding the 6th Bn Welsh Regiment."
