OK. I've started an expansion of this page, including:
- Removing, for the present, the definition of "government by all." This does not seem to be a common usage. Neither the OED nor any of the other print dictionaries I consulted include it. A quick survey of Google Books yielded no systematic use of the term other than in reference to Andrews' theory.
I'll have to dig into my files and the secondary literature on Andrews to establish things like the dates for the Woodhull and Claflin's Weekly "Bulletins." I'll work on this as time allows. Libertatia 19:16, 12 October 2007 (UTC)
