It was Bam Bam Bigelow that made it famous. The first incident was on an ECW TV taping prior to Hardcore Heaven '97. This sentence - particularly the part in bold type makes no sense

Matt HysonSpike Dudley – Hyson wrestled as "Spike Dudley" for the vast majority of his career and is now retired. A search of the Google news archive returns considerably more results for "Spike Dudley" than for "Matt Hyson". Hyson is, objectively, better known by this name.
