I usually refer to the principal character of the television series Doctor Who as Doctor Who. I know that usage comes and goes, and often it has been common practice to refer to him as "The Doctor", but I've called him Doctor Who since 1963 and am unlikely to change now.

He was often called "Doctor Who", I believe, in the credits of television episodes in the sixties and seventies, although the characters within the programme tended to refer to him as "The Doctor". But to me he has always been Doctor Who--if only because it's natural, when referring to him in conversation, to identify him in a way that will be easily recognisable. "Doctor Who would find a way out of this situation" is less awkward than "The Doctor would find a way out of this situation" followed by a brief conversation explaining which doctor one is referring to.
