The song is about addiction, plain and simple. Anyone that has ever stuggled with drugs or alcohol will find this to be obvious. 'My Sweetest Friend' is the object of addiction.

Well the downward spiral is about Charles manson and psychopathy. A helter skelter being a downward spiral for instance. Addiction to narcissim in may well be about.
